How to fill in the scantron answer sheet

These are general directions for students while filling out scantron answer sheet:

• Use a pencil (no. 2) 

• Do not use ink or ballpoint pens 

• Do not fold the answer sheet 

• Questions will be marked wrong when a student leaves a question unanswered (blank). 

• Completely fill in each circle 

• Normally, you MUST fill 5 sections in the scantron answer sheet: 

1. Name 

2. Date 

3. Course number 

4. Student number 

5. Answer section (A, B, C, D, E) 

• Print your name as (LAST <space> FIRST). Then fill in each circle that corresponds to the letter above it. Fill in one letter of your name in each column. 

• Do the same for date, course number, and student number 

• If you are erasing a mistake, then completely remove all pencil marks from the incorrect answer. Do not put X through it. 

• Do NOT write anything above the red bar of the red test scantron sheets
