To submit request to reset your Crystal Reports password, please click on Open Ticket button on the right. This will bring up a form that you will need to fill-out and submit to open a support ticket with the I.T. Service Desk. You will receive a confirmation e-mail with the ticket reference number and additional instructions.

UWin Account Password
Please note that you should not use this form to request your UWin ID password to be reset. You can reset your own UWin password using your security questions. However, if you do not know the answers to your security questions, you must visit I.T. Service desk in person and present your ID to have your UWin ID password reset. Once this was done, you should configure your security questions (Shared Secrets) so that you can use the self-service option in the future.
