Scantron Student Evaluation of Teaching (SET)

The Scantron software and hardware is used for computerized Student Evaluation of Teaching (SET) in I.T. Services.  Please record the ticket ID that is assigned to the request on the envelope that you submit as well as for your personal records.

Requesting Assistance

There is now a new way to fill out and submit the form associated with the package being submitted for SET. Instead of filling out a paper form at the IT Services counter, a ticket must be opened by filling out an on-line service request form prior to coming to Operations Service Counter.

To access the service request form, please click the Open Ticket button on the right. This will bring up a form that you will need to fill-out and submit to open a support ticket with the I.T. Service Desk. You will receive a confirmation e-mail with the ticket reference number and additional instructions.

The Number of Classes field allows Operations to verify that the counts submitted match and investigate at once if they do not.  Returned has to do with whether you want the forms returned to you or not.  The Academic Term allows us to track resubmissions.
