Self enrollment has been disabled in Qualtrics to attach additional governance and auditing to the process.
Please open a ticket here Service - Qualtrics Surveys (teamdynamix.com) to request a new account.
Once your account has been created, you will go to uwindsor.qualtrics.com to log in (do not go to the generic log in page at www.qualtrics.com. Accounts created there are not under the University's account and we have no control over them).
NOTE: There is still a link to self enroll on uwindsor.qualtrics.com, but it will take you to www.qualtrics.com and you can only get a generic, very limited free account.
